These tools and resources were created by an interprofessional team at UC Davis Health System and funded in part by the Robert Wood Johnson Foundation. The project, Nurse-Led Intervention to Facilitate Patient Activation for Improved Pain Self-Management (2009-2011), involved experts in nursing, psychology and pain management who worked in collaboration with people undergoing spine surgery at UC Davis Spine Center.
