By my signature below, I accept an appointment at the PGY-1 preliminary level, in the Department of Surgery, University of California, Davis Health System beginning June 25, 2016 - June 30, 2017. I understand that a formal contract from University of California, Davis Health System (UCDHS) Human Resources will be sent at a later date.

I also understand that until this letter of agreement is received in the Surgery Residency Office, either by fax (916-734-5633) or email (jlbraxton@ucdavis.edu), an electronic signature is binding as well, the position offered to me might be offered to other applicants. Acceptance into the program will be granted to the applicant who returns this letter to us first.
