Current UC Davis medical students

Requests for enrollment verification, acting internship applications, clerkship letters, name changes, credit by exam, course waiver, PELP, ID badges, adviser changes, etc. can be dropped off or mailed to the School of Medicine Registrar's Office.

UC Davis medical school graduates

Requests for degree verification, certification of diploma, and completion of licensure forms should be made with the Authorization for Release form and may be mailed or faxed to the School of Medicine Registrar's Office.
