The UC Davis Lung Cancer Integrated Service Line Streamlines Access to Our Resources
Physicians may easily refer patients to the UC Davis Thoracic Oncology Program. Our Lung Cancer Integrated Service Line (ISL) is committed to transparent, rapid communication with outside referring providers. We have lung cancer nurse navigators and support staff members who are committed to helping patients get world-class care in a timely fashion. Our multidisciplinary team of lung cancer leaders across medical and surgical specialties focuses on managing the most challenging lung cancer cases and rarer thoracic malignancies.

Making a Referral
Referring physicians who wish to request UC Davis Thoracic Oncology Program services for their patients can use our electronic referral platform
You may download a referral form. Please submit a completed referral form with a clear diagnosis and CPT code, front and back images of all of the patient’s insurance cards, authorization from the patient’s insurance provider, and any completed lab reports, images, medical reports, medical notes from the past 30 days.
