Best method is to email your request to hs-studentrecords@ucdavis.edu. Requests for enrollment verification, acting internship applications, clerkship letters, name changes, credit by exam, course waiver, PELP, ID badges, etc. can be dropped off or mailed to the School of Medicine Registrar's Office. Please allow up to 5 business days to process your request.

UC Davis Medical School Graduates
Best method is to email your request and release form to hs-studentrecords@ucdavis.edu. Requests for degree verification, certification of diploma, and completion of licensure forms should be made with the Authorization for Release form and may be mailed or emailed to the School of Medicine Registrar's Office. Please allow up to 5 business days to process your request.

Note: The UC Davis SOM Registrar’s Office cannot verify Residency or Fellowship program completion.  Please contact the Clinical Department in which the physician completed their residency or fellowship for this information.
