The Office of Student Records handles the following for UC Davis School of Medicine graduates:

  • M.D. Degree Verification
  • Certification of Diploma
  • Completion of Licensure Forms
  • Copies of Dean's Letter
  • Ordering of Replacement/Duplicate Diploma

Official medical school transcripts are ordered from:

All services of the records office are free of charge. All requests must be accompanied by an Authorization for Release (PDF).
