Services, resources, and support for individuals with disabilities and their families may change as people move from childhood to adult life. Transition is the word used to describe movement and change from one phase of life to another and is often used to refer to the time period from 18 years old to adulthood.
The resources included on this page have helpful information about transition and adult life. If you know of additional resources that may be helpful to the community, please send your suggestions to sharon.galloway@ucdmc.ucdavis.edu.
