Michael K. Hooper, M.D., F.A.A.F.P.
Medical Director
Dr. Hooper became the Medical Director of Health Management and Education (HME) in 2016. He has been with UC Davis since 1996, initially as a Family Physician in the Primary Care Network (PCN) and then transitioned to an administrative role in the PCN in 2012 when he also took on the role as Medical Director for UCD Managed Care. In his present position as the Medical Director for Care Services and Innovation, one of his many responsibilities is to oversee HME. Dr. Hooper believes an important part of his role is to support and help guide the efforts of the dedicated, talented group of professionals in the Health Management and Education Department. Dr. Hooper considers the work in HME a very important part of what UCD is striving for -  to improve the treatment and health outcomes of the patients. As a Family Physician with experience in clinical practice for over 25 years, he feels strongly that the primary care physician plays a key role in assuring that their patients receive high quality medical care across the spectrum of their life and the continuum of care. The primary care physician role has become increasingly complex for a variety of reasons especially caring for the sickest, most vulnerable patients managed by the primary care physicians. Dr. Hooper looks forward to the privilege of working with the HME team for the benefit of the patients cared for by all of our UCD physicians.

Nicole Hansen

Nicole Hansen, R.N., M.S.N., N.E.A-B.C.
Manager

Nicole oversees the Ambulatory Case Management, Education, and the Hospital Discharge Call Programs. She promotes interdisciplinary collaboration to optimize care coordination, disease management, and patient-centered quality care. Nicole began her career at UC Davis Health in 2002. She completed her undergraduate nursing degree at Samuel Merritt College, earned a Master of Science in Nursing from Duke University and is an ANCC Board Certified Nurse Executive.
Kelly Grady

Kelly Grady, M.S.N, M.P.A, R.N-B.C, P.H.N., O.N.C.
Hospital Discharge Call Program Supervisor

Kelly is the coordinator for the Hospital Discharge Call Program which launched at UC Davis Health in September 2016. This program is an outreach effort by registered nurses to patients discharged from the hospital to home. Kelly joined UC Davis in 2014 providing telephone triage services for primary care as well as outpatient Cardiology and Nephrology clinics. Kelly has diverse clinical experience working with orthopedics, trauma, neurology and cardiovascular patients. Kelly has her Master of Science in Nursing Leadership, a Master of Public Administration and a Bachelor of Science in Business. She is a licensed Public Health Nurse and has her Orthopedic Nurse Certification and Cardio-Vascular Nurse Certification.

Monica Delson

Monica Delson, R.N., B.S.N., MSN-NE.d.
Case Manager Supervisor

Monica is bilingual in Spanish and brings a diverse background to her case management Supervisor nursing role in Health Management and Education. Her experience includes patient chronic disease education, care management, outpatient case management, as well as working with  population health initiatives ; other experience includes working in emergency/ trauma and inpatient medical surgical units. She also works with high risk populations and provides chronic care management education to physicians and nurses. Additionally, she has worked on the development of Medicare and Medi-Cal quality improvement metrics and initiatives. Monica is passionate about cultivating a culture of excellence and providing quality patient care to diverse-high risk patient populations.
Glee Van Loon

Glee Van Loon, R.D., C.D.E.
Education Supervisor
Glee is a Registered Dietitian and Certified Diabetes Educator who has been with Health Management and Education department since 2007. As part of department leadership, Glee oversees the education team, contributes to the department’s overall growth and development and ensures the delivery of high quality education. Glee has been a dietary specialist in diabetes management since 2003. She also facilitates health classes and provides individual medical nutrition therapy.

Anita Watson

Case Manager Supervisor

Anita is a Registered Nurse and a Case Manager Supervisor for Health Management and Education. She has over 30 years of nursing experience helping patients with chronic conditions improve their health through better self-care. Anita joined UC Davis in 2006 in the Managed Care department as a care manager and then supervisor. In 2016 Anita brought her leadership, knowledge, and clinical experience to Health Management and Education where she continues to promote and support patient wellness and self-care.

Stephanie Guerrero

Stephanie Guerrero

Stephanie brings over 7 years’ experience to her role as an Administrative Assistant. She joined Health Management and Education in 2017 and provides global office support as well as acts as a department resource for UC Davis patients and staff. Stephanie has a medical assisting certification and plans to pursue a degree in registered nursing.

Dusten Hickman
Project Policy Analyst III
Dusten joined HME in 2018. He has a background of over 8 years working in the health care field in different capacities. His background is with healthcare IT and has experience working on many different aspects of the Epic EMR utilized here at UC Davis and other organizations. He has supported and conducted training for many different aspects of healthcare and has worked with physicians, nurses, care managers, internal staff and patients providing them with the necessary tools and training they need. In his current role he will be providing analytical support to the education team and department staff as well as assisting with project development and reporting functions.

Pamela Jones White

Pamela Jones-White
Administrative Officer II

Pamela has worked at UC Davis for nearly 20 years. Prior to joining Health Management and Education in 2016 Pamela was a Managed Care Specialist. She gained extensive knowledge and experience in utilization management, insurance and authorizations. In Health Management and Education, Pamela utilizes her experience as an analyst to collaborate with the case management team and provides excellent service for the patients of UC Davis Health.

Kristina has worked at UC Davis Health providing quality customer service and departmental support since 2002. She joined the department in 2011 as a Senior Community Health Program Representative, maintaining the daily operations of the education team and supporting the efforts of the Director. In her current role Kristina provides analytical and data management support as well as she assists staff with project development and website support.

Health Educator
Judy came to Health Management and Education in 2012 as a Health Educator. Her previous work in corporate health promotion adds tremendous value to the education team. She facilitates health classes on topics such as tobacco cessation, weight management, diabetes, stress management, exercise, and blood pressure. Judy also promotes learning through health fairs, newsletters, and other educational projects.

Moreen Sharma

Health Educator
Moreen is a Certified Health Education Specialist who has been with UC Davis Health since 2012. She brings dedication to the work she does while promoting, maintaining and improving our classes and programs. Moreen is a compassionate health educator who facilitates self-management classes for people with chronic conditions. She is also currently working toward becoming a Tobacco Treatment Specialist.

Debra Fishman

Debra Fishman, PsyD.

Dr. Fishman is a Psychologist who completed her clinical training as an Intern and Fellow with UC Davis, Department of Family and Community Medicine. She provides individual counseling and group education to patients with chronic health conditions. Dr. Fishman’s work is focused on helping patients learn about and integrate specific health self-management tools and strategies into their personal health management plan. She is valued for her broad background that includes training and work experience in Health Psychology, Exercise Physiology, Cardiac Rehabilitation, Ergonomics, and management of outpatient physical therapy services. She is dedicated to providing patient-centered care.

Diana Grabchuk

Diana joined UC Davis Health in 2016. She is a Registered Nurse with the Hospital Discharge Call Program. In her role Diana assists patients to have a smooth transition from the hospital to their home by providing telephonic resources, education and support. Diana’s passions include helping patients to optimizing their health and quality of life. Diana’s nursing experience includes medical surgical nursing and cardiac transition care. She has a Bachelor of Science in Nursing.

Trish Smith

Registered Nurse
Trish is a Registered Nurse with the Hospital Discharge Call Program who assists patients in their transition from the hospital environment to their home. Trish has worked for UC Davis Health for more than 19 years. Her vast experience includes hospital neurology, cardiac and general medicine intensive care units and outpatient cardiology and nephrology clinics. Trish has a Bachelor of Science in Nursing and is also is a Certified Cardio-Vascular Nurse.

Rosemarie Cosgayon

Rosemarie Cosgayon, Pharm.D., M.S.
Case Manager
Rosemarie joined the HME team as a Pharmacist in 2018 to help expand the blood pressure program and to assist patients with medication management needs. Her experience includes working in the ambulatory care clinic setting where she offered pharmaceutical services including medication and chronic disease management for anticoagulation and diabetes. In addition, she has experience in transition of care clinics, specifically with patients with chronic obstructive pulmonary disease.

Linda Phan

Linda Phan, Pharm D., B.C.P.S.
Case Manager
Linda has been a Pharmacist for over 20 years, most of which have been spent at UC Davis taking care of hospitalized patients. Linda joined the HME team in 2014 to work more closely with patients on their needs beyond the hospital. In addition to her Doctorate in Pharmacy, Linda has additional training in both geriatric pharmacy practice and is currently board certified as a pharmacology specialist.

As a Ambulatory Case Management Pharmacist, Huong’s experience includes expertise in long term care, managed care and clinical health education. She joined Health Management and Education in 2015 and is collaborating with an expanded care team to initiate an innovative program for guiding patients with high blood pressure to better control.

Marie Barone

Marie Barone, R.D., C.D.E., C.S.O.W.M.
Educator
Marie is a Registered Dietitian and Certified Diabetes Educator who also holds a specialty certification for obesity weight management. She has been with UC Davis Health for more than 10 years. Serving as a nutrition expert for the Health Management and Education team she facilitates self-management classes for patients with chronic conditions, provides individuals with medical nutrition therapy, and takes lead in program development for the education team, including electronic education.

Melinda Gong

Melinda is a Registered Dietitian, Certified Diabetes Educator as well as a Certified Specialist in Weight Management. As a seasoned Dietitian she has worked with adult and pediatric patients in a hospital setting. She joined UC Davis Health in 2012 as a nutrition expert and educator for patients with chronic diseases. Melinda facilitates group health classes and works with patients individually for nutrition management.

Alexis Blount

Alexis Blount, R.N., F.N.P., C.D.E.
Educator
Alexis is a Registered Nurse, board certified Family Nurse Practitioner, and Certified Diabetes Educator. She joined the Health Management and Education team as a diabetes educator and tobacco treatment nurse in 2018. She has worked over thirty years as a registered nurse in the fields of behavioral health, administration, and chronic disease management. Her lifelong interest in how health systems can promote patient centered care has led her to pursue a Ph.D. in integrative health and health systems.

Wendy Braccio

Wendy is a Certified Case Manager and has been a Registered Nurse since 1982. Her diverse background includes women and children’s health, oncology, orthopedics, surgery and case management. In 2017 Wendy brought her wealth of experience to Health Management and Education where she dedicates time to improving the health and wellbeing of UC Davis Health patients.

Gabby is a Registered Nurse specialized in diabetes education. Her nursing experience includes medical surgical nursing and nurse coordination for weight loss surgery patients. Gabby’s passions include assisting patients with chronic conditions to optimize their health and improve their quality of life through group and individualized health education. Emily Murdock

Emily Murdock, R.N., B.S.N., P.H.N.
Case Manager
Emily is a Registered Nurse Case Manager providing telephonic support to patients and families. She earned a Bachelor of Science in Nursing from Samuel Merritt University and is poised to complete her Masters in Nursing Case Management in August 2018. Emily has a background working in cardiac intensive care, medical-surgical, urgent care, triage, and hospice nursing. She has worked for UC Davis since 1998, and has served a term on the Yolo Hospice Board of Directors. She volunteers as a Diversity Educator for Campus Community Relations, and currently serves on the UC Davis Health Ambulatory Nurse Governance Council.

Sharon is a Registered Nurse and is specialty certified as a Certified Heart Failure Nurse. She has her Master’s degree in Counseling & Psychology as well as Exercise Physiology. She has worked for UC Davis Health for over 15 years. Sharon is a lead facilitator of the heart failure self-managing class.

Andrea Quinonez

Andrea is a Registered Nurse with experience in outpatient care, intensive care, community health, and research. She joined UC Davis Health in 2011 and has worked in a variety of ambulatory care settings. Andrea’s passions include working with people “where they’re at” to improve their overall health. While earning her Doctorate and Masters in Nursing, she explored ways to support positive changes in physical activity, nutrition, and other self-care behaviors, both on an individual and community-wide basis. Andrea’s role with the Health Management and Education team includes quality improvement, group education, Hospital Discharge Call Program and project facilitation on a variety of health topics

Kate is a Registered Nurse with Health Management and Education. She has worked as a nurse since 1981 in Arizona, Nevada and California in various settings including inpatient chronic and acute care, medical transport, and outpatient oncology, clinical trials, education, managed care and case management. She has been at UC Davis since 2011 in oncology clinical trials and the Managed Care Department and Health Management and Education Department since 2013. She is aware of the many challenges in the health care system and is passionate about working individually with patients to meet their needs. Kate wants patients to know the high standards and caring professionalism that UC Davis Health provides as well as the resources available to assist with the management of their health care.

Linda is a Licensed Clinical Social Worker who joined Health Management and Education’s Ambulatory Case Management team in 2013. Earlier in her UC Davis Health career Linda facilitated breast cancer awareness and education as a Senior Mammographer. She has worked at UC Davis Health for more than 30 years in various clinical capacities. In her role as a Clinical Case Manager Linda’s extensive experience allows her to address a variety of complex and diverse mental health conditions.

Daniel is a Licensed Clinical Social Worker and utilizes his more than 20 years of case management and clinical mental health experience in his role as a case manager. He has been a member of the Ambulatory Case Management team since its launch in 2012.

Emily is a Licensed Clinical Social Worker who joined the HME team in 2018. Emily brings more than 10 years of social work and case management experience. She has worked in a variety of settings including skilled nursing facilities and inpatient hospital units. While working in these settings Emily gained experience working with a diverse group of populations including pediatrics, adult medicine, and geriatrics.

Katie is a Case Manager with a Master degree in Social Work. Her career at UC Davis Health began in 2002 providing community case management and clinical case management for discharge planning. She joined Health Management and Education’s Ambulatory Case Management team in 2012. Her background and training bring valuable skills to her current role as a case manager, intake coordinator and resource expert.

Stephanie is a Licensed Clinical Social Worker who joined the Health Management and Education team in 2017. She brings over 10 years of experience in clinical mental health. Prior to joining Health Management and Education Stephanie specialized in trauma and abuse, working with children and families to provide therapy and case management. Stephanie is also a veteran who retired from the U.S. Air Force Reserves in 2012.
