Our staff provides each identified primary care and referring physician notification of their patient’s admission to or discharge from the medical center, along with any available clinical information regarding their patient’s hospital stay. If you need assistance obtaining information on your patient’s admission, discharge treatment plan or care while hospitalized at UC Davis Medical Center, please contact us.
