Ambulatory Care - Transitions of Care

NEW PROGRAM
The University of California, Davis Health System, Department of Pharmacy offers a one-year PGY-2 Ambulatory Care Residency with an emphasis in Transitions of Care.

Goals:

• To learn and participate in the process of developing the new pharmacy transition of care across the UC Davis Health System.

Rotations:
The residency will be tailored to suit the resident’s individual goals and interests. The resident will spend much of the year with the Ambulatory Care Team learning patient care and common pitfalls after discharge, then they will spend a core 3-month rotation in Transition of Care working toward improvements in the discharge process.  They will also have required rotations in some crucial inpatient areas, such as Cardiology.

• Participates in the ground floor of the Transition of Care Team -- starting 2013
• Possible electives in the areas of Formulary Management, Faculty Practice, Ambulatory Care Management, Informatics, etc.
• Longitudinal Experience with Medical Student-Run Clinics
• Opportunity for International Rotation if requested
• Currently Existing Ambulatory Care Clinics (Pharmacist-Run)
  • Anticoagulation
  • Cardiology
  • Refill (Longitudinal)
  • Medication Therapy Management (separate rotation)
  • CARES HIV (separate rotation)
  • Possible CHF experience

PROJECTS
Major project will be pre-set to explore the outcomes of the Transition of Care Team for possible expansion.

• Residency project to be presented at Western States Conference
• Presentation of new protocols for the Pharmacy & Therapeutics Committee
• MUE project

TEACHING
The resident will be responsible for assisting preceptors with clerkship teaching for pharmacy interns from University of California, San Francisco and University of the Pacific. In addition, the resident will have an opportunity to participate in staff development and education (nursing, housestaff, pharmacy students, and medical students).  PGY-2 applicants who do not have a Teaching Certificate will have the opportunity to get one during the residency.

ADDITIONAL RESPONSIBILITIES
The resident will be required to fulfill service hours in Transitions of Care and Medical Student-Run Clinics. The resident will also be part of the Anticoagulation Clinic after-hours critical pager schedule.

STIPEND
Approximately $48,450 (paid in monthly increments)

PERIOD OF APPOINTMENT
August 2013 – July 2014

ELIGIBILITY
The applicant shall be licensed in California or be eligible to take the California State Board Exam for pharmacy licensure in June 2013. An on-site interview is required.
