Once you are logged on, go to the "Service Request Form". A page already populated with your name will pop up.

Sacramento Researchers:
Deliver your specimen to Histology Laboratory, PATH Building, UCDMC, 4400 V Street, Sacramento, 2nd Floor, Reception Desk. We always appreciate a call to alert us regarding incoming samples. Please contact Irmi Feldman (4-3026) prior to sample delivery.

UC Davis Researchers:
Deliver your specimen to Medical Pathology and Laboratory Medicine Office, 3455 Tupper Hall. A courier service will pick up specimens and deliver slides. We always appreciate a call to alert us regarding incoming samples. Please contact Trudy Aigner (752-2710) prior to sample delivery.

There will be a log in both locations. Please complete the log. You will need your order number from the Cancer Center website when logging in your specimen.
