Clinical Laboratory Science Training Program: Application Procedure
- Obtain an all application materials by clicking the links below:
- Application Form
- Recommendation Letter Form (You will need 3 copies)
- Instruction on how to write a personal statement
- Survey Form
Please note: If you do not have Adobe Acrobat Professional software on your PC, you will be able to print but not save this form.
- Submit the following:
- A completed and signed standardized application form
- A personal statement of interest
- Official transcripts (sent directly from the Office of Registrar)
- Three recommendation letters (sent directly from the instructors or supervisors with any combinations of instructors and supervisors)
- A Survey form (optional)
- Submit to:
