Example #1:

"I would like to nominate Lauren Rountree in the category of Professional relationships and Teamwork for her expertise as a New Graduate Nurse Residency Program facilitator and Preceptor. Lauren served as both my new graduate facilitator and preceptor while simultaneously exceeding her roles as a CNIII, the chair of the Unit Practice Council, an instructor for various new graduate classes, and a Master’s student at the Betty Irene School of Nursing.

Being the first new graduate on MSICU Blue, Lauren fully embraced her role as my preceptor by challenging my critical thinking skills, demonstrating exceptional leadership qualities, and assisting me with the development of a comprehensive skill set- all of which are imperative to the success of an ICU nurse. On a daily basis, we assumed care of the most critically-ill, medically-demanding patients to maximize my opportunity for the most challenging and comprehensive nurse-patient situations. To further enhance my understanding of complex critical care interventions, Lauren always went above and beyond to search for clinical resources and new learning opportunities outside our patient assignments. She represents the face of EBP and research to validate patient care. Her strong direction and focus have become invaluable to me.

Beyond her role as a preceptor, Lauren was also an exceptional facilitator who provided a stress-free transition into the culture and social environment of MSICU Blue. Lauren continuously enhanced my development into a professional nurse both with my patients and colleagues. She helped me become a member of the Unit Practice Council and continues to support my endeavors with precepting nursing students. Lauren is always the person I can go to when I need assistance with an overwhelming patient assignment or simply need a friend after an emotionally demanding shift.

Lauren displays ongoing support, education, and leadership everyday- never failing to create a positive learning environment on the unit. She serves as an extraordinary leader and mentor for me as a new graduate- demonstrating the values of compassion, advocacy, and continued education amongst both her patients and colleagues. She fully embodies the qualities I strive to exemplify in my future practice as a nurse leader and mentor. Lauren is an attestation to the true meaning of "exemplary preceptor" and what it means to be a UCD nurse! It is with the highest honor and gratitude that I recommend her for this award!"

Example #2:

“Denise helps us transition patients who are undergoing a life changing surgery. She prepares them for their ostomy surgery. These patients usually have been newly diagnosed with cancer or other life threatening diseases and teaches them what to expect when they are facing an ostomy surgery. She has classes that inform them about their ostomy surgery, marks the site where the ostomy will be placed and is very thorough in her education. These patients seem to accept having an ostomy because of her education. I am always thankful that they have attended Denise's classes because they are knowledgeable about their ostomy. These patients are fully informed about what to expect after the surgery, my role has been made simpler in teaching them about their ostomy. Most of the patients who have attended the pre-op classes with Denise are more accepting of their ostomy and they are so appreciative that Denise has helped them with this life changing surgery. What great teamwork when we are able to help patients be successful.”

“Wendy Wong is an exceptional RN but I would like to recognize her because she goes above and beyond to help her fellow teammates. She is always coming in extra to help with breaks or take on an assignment when we need her the most. She is always ready to help and take on any task that you ask her too. Wendy is always willing to teach and make sure you are doing things per policy and for the patients' best interest. She is always very approachable and ready to work hard. Wendy never leaves your side if your patient is critical or unstable. She gives you reassurance and makes sure you know she is there for you. She is a team member you can always depend on. I always feel a sense of relief when ever I know I am working with her side by side.”
