• Download these forms by clicking on the desired format: MS Word or Adobe PDF. If you wish to use the PDF format, you must have Adobe Acrobat Reader, which is free at Adobe's site, installed on your computer.
• These links, with the exception of PDF, make a file on your hard drive. You can either save them on your computer's hard drive, a floppy disk, or open them immediately. Some browsers, especially Explorer 4 & 5, will open a viewer window rather than launching the word processing application. In this case you will need to "save as" instead of open. In some cases it may even be necessary to "right-click" the link rather than opening it.
• Once the document is saved, you can use your word processor to fill in the blanks, or print the blank form and complete it on paper (all formats).
• Please do not change the text of the forms, other than to type in your answers. However, you may have to modify the spacing and/or page breaks to accommodate your make and model of printer.
