Medical Director
Dr. Hooper became the medical director of Health Management and Education (HME) in 2016. He has been with UC Davis since 1996, initially as a Family Physician in the Primary Care Network (PCN) and then transitioned to an administrative role in the PCN in 2012 when he also took on the role as Medical Director for UCD Managed Care. In his present position as the Medical Director for Care Services and Innovation, one of his many responsibilities is to oversee HME. Dr. Hooper believes an important part of his role is to support and help guide the efforts of the dedicated, talented group of professionals in the Health Management and Education Department. Dr. Hooper considers the work in HME a very important part of what UCD is striving for -  to improve the treatment and health outcomes of the patients. As a Family Physician with experience in clinical practice for over 25 years, he feels strongly that the primary care physician plays a key role in assuring that their patients receive high quality medical care across the spectrum of their life and the continuum of care. The primary care physician role has become increasingly complex for a variety of reasons especially caring for the sickest, most vulnerable patients managed by the primary care physicians. Dr. Hooper looks forward to the privilege of working with the HME team for the benefit of the patients cared for by all of our UCD physicians.

Judith Johnson

Judith Johnson, D.N.P., M.S.N., R.N., N.E.A.-B.C.
Director
Judith is the Director of Health Management and Education and has worked for UC Davis Health since 1988. She has been with the Primary Care Network since 2003, becoming the Director of HME in 2016. She brings extensive nursing experience including work with patients with chronic illnesses, culturally diverse populations, and those with psychosocial and mental health needs. As the new Director of Health Management and Education, Judith brings a dedication to patient centeredness and foresees the future growth of the HME team in continuing to promote quality care and care coordination while empowering patients with the tools they need for success and wellness.

Kelly Grady

Kelly Grady, M.S.N, M.P.A, R.N-B.C, P.H.N, O.N.C
Hospital Discharge Call Program Supervisor

Kelly is the coordinator for the CipherHealth Voice post-discharge call program which launched at UC Davis in September 2016. The CipherHealth program is an outreach effort by UC Davis Registered Nurses to patients discharged from the hospital to home. Kelly has worked for UC Davis since 2014 providing telephone triage for the PCN Clinics and, more recently, working as a clinical triage nurse in the outpatient Cardiology and Nephrology Clinic. Kelly has diverse clinical experience working with orthopedics, trauma, neurology and cardiovascular patients. Kelly has her Master of Science in Nursing Leadership, a Master of Public Administration and a Bachelor of Science in Business. She is a licensed Public Health Nurse and has her Orthopedic Nurse Certification and Cardio-Vascular Nurse Certification.

Nicole Hansen

Nicole Hansen, R.N., M.S.N., N.E.A-B.C
Case Manager Supervisor

Nicole began her career at UC Davis Health in 2002 and currently oversees the care delivered by the Registered Nurse, Social Work and Pharmacy Care Managers. Nicole has a unique nursing and leadership background. Prior to joining HME in 2017, she was the associate director for a patient-centered clinical research program and clinic. She was a founding board member of the International Association of Clinical Research Nurses, an ANA recognized professional nursing organization for clinical research nurses; a founding board member of Friends of International Clinical Research Nursing, a foundation that supports scholarship and professional development of clinical research nurses; a scientific reviewer for the Institutional Review Board; a mentor to graduate students, and guest lecturer locally and abroad. She completed her undergraduate nursing degree at Samuel Merritt College, earned a Master of Science in Nursing degree from Duke University and is ANCC Board Certified Nurse Executive.
Glee Van Loon

Glee Van Loon, R.D., C.D.E.
Education Supervisor
Glee is a registered dietitian and Certified Diabetes Educator (CDE) who has been with Health Management and Education department since 2007. As part of department leadership, Glee oversees the education team, contributes to the department’s overall growth and development and ensures the delivery of high quality education. Glee has been a dietary specialist in diabetes management since 2003. She also facilitates health classes and provides individual medical nutrition therapy.

Anita Watson

Case Manager Supervisor

Anita is a Registered Nurse and a Case Manager Supervisor for Health Management and Education. She has over 30 years of nursing experience helping patients with chronic conditions improve their health through better self-care. Anita joined UC Davis in 2006 in the Managed Care department as a care manager and then supervisor. In 2016 Anita brought her leadership, knowledge, and clinical experience to Health Management and Education where she continues to promote and support patient wellness and self-care.

Sarah Alvarez

Sarah joined UC Davis Health in 2017 as a Health Professions Education Specialist. She is a certified medical secretary with over 17 years experience. She has supported patients and care teams in a variety of health system settings throughout northern California. In her current role Sarah’s expertise is used to coordinate the daily operations of the education team. Additionally she is a key resource for patients and instrumental in scheduling them for self-management classes. She participates in a variety of department projects and prides herself in delivering exceptional service.

Stephanie Guerrero

Stephanie brings over 7 years’ experience to her role as an administrative assistant. She joined Health Management and Education in 2017 and provides global office support as well as acts as a department resource for UC Davis patients and staff. Stephanie has a medical assisting certification and plans to pursue a degree in registered nursing.

Derika Faamausili

Project Policy Analyst III
Derika has been with UC Davis Health for over 10 years. She joined the Health Management and Education team in 2012, and currently supports analysis and data management, patient referral processes, coordination of patient education needs, and complex patient queries.  Derika brings diverse life coaching skills and holistic health knowledge to her role.

Pamela Jones White

Administrative Officer II

Pamela has worked at UC Davis for nearly 20 years. Prior to joining Health Management and Education in 2016 Pamela was a Managed Care specialist. She gained extensive knowledge and experience in utilization management, insurance and authorizations. In Health Management and Education, Pamela utilizes her experience as an analyst to collaborate with the case management team and provides excellent service for the patients of UC Davis Health.

Kristina McCoy

Kristina has worked at UC Davis Health providing quality customer service and departmental support since 2002. She joined the department in 2011 as a Senior Community Health Program Representative, maintaining the daily operations of the education team and supporting the efforts of the Director. In her current role Kristina provides analytical and data management support as well as she assists staff with project development and website support.

Judy Hoppezak

Judy Hoppezak, M.S.
Health Educator
Judy came to Health Management and Education in 2012 as a Health Educator. Her previous work in corporate health promotion adds tremendous value to the education team. She facilitates health classes on topics such as tobacco cessation, weight management, diabetes, stress management, exercise, and blood pressure. Judy also promotes learning through health fairs, newsletters, and other educational projects.

Moreen Sharma

Moreen Sharma, B.S., C.H.E.S.
Health Educator
Moreen is a Certified Health Education Specialist who has been with UC Davis Health since 2012. She brings dedication to the work she does while promoting, maintaining and improving our classes and programs. Moreen is a compassionate health educator who facilitates self-management classes for people with chronic conditions. She is also currently working toward becoming a Tobacco Treatment Specialist.

Health Psychologist

Debra Fishman

Debra Fishman, PsyD.
Health Educator
Dr. Fishman is a Psychologist who completed her clinical training as an Intern and Fellow with UC Davis, Department of Family and Community Medicine. She provides individual counseling and group education to patients with chronic health conditions. Dr. Fishman’s work is focused on helping patients learn about and integrate specific health self-management tools and strategies into their personal health management plan. She is valued for her broad background that includes training and work experience in Health Psychology, Exercise Physiology, Cardiac Rehabilitation, Ergonomics, and management of outpatient physical therapy services. She is dedicated to providing patient-centered care.

Hospital Discharge Call Program

Diana Grabchuk

Diana Grabchuk, R.N., B.S.N
Registered Nurse
Diana joined UC Davis Health in 2016. She is a registered nurse with the hospital discharge call program. In her role Diana assists patients to have a smooth transition from the hospital to their home by providing telephonic resources, education and support. Diana’s passions include helping patients to optimizing their health and quality of life. Diana’s nursing experience includes medical surgical nursing and cardiac transition care. She has a Bachelor of Science in Nursing.

Trish Smith

Trish is a hospital discharge call program nurse who assists patients in their transition from the hospital environment to their home. Trish has worked for UC Davis Health for more than 18 years. Her vast experience includes hospital neurology, cardiac and general medicine intensive care units and outpatient cardiology and nephrology clinics. Trish has a bachelor of science in nursing and is also is a certified cardio-vascular nurse.

Pharmacist

Linda Phan

Linda Phan, Pharm D., B.C.P.S.
Case Manager
Linda has been a pharmacist for over 20 years, most of which have been spent at UC Davis taking care of hospitalized patients. Linda joined the HME team in 2014 to work more closely with patients on their needs beyond the hospital. In addition to her Doctorate in Pharmacy, Linda has additional training in both geriatric pharmacy practice and is currently board certified as a pharmacology specialist.

Huong Sandhu

As a Ambulatory Case Management pharmacist, Huong’s experience includes expertise in long term care, managed care and clinical health education. She joined Health Management and Education in 2015 and is collaborating with an expanded care team to initiate an innovative program for guiding patients with high blood pressure to better control.

Marie Barone

Educator
Marie is a Registered Dietitian and Certified diabetes Educator who also holds a specialty certification for obesity weight management. She has been with UC Davis Health for more than 10 years. Serving as a nutrition expert for the Health Management and Education team she facilitates self-management classes for patients with chronic conditions, provides individuals with medical nutrition therapy, and takes lead in program development for the education team, including electronic education.

Melinda Gong

Educator
Melinda is a Registered Dietitian, Certified Diabetes Educator and trained in adult weight management. As a seasoned Dietitian she has worked with adult and pediatric patients in a hospital setting. She joined UC Davis Health in 2012 as a nutrition expert and educator for patients with chronic diseases. Melinda facilitates group health classes and works with patients individually for nutrition management.

Wendy Braccio

Wendy Braccio, R.N., M.S.N., C.C.M.
Case Manager
Wendy is a Certified Case Manager and has been a Registered Nurse since 1982. Her diverse background includes Women and Children’s Health, Oncology, Orthopedics, Surgery and Case Management. In 2017 Wendy brought her wealth of experience to Health Management and Education where she dedicates time to improving the health and wellbeing of UC Davis Health patients.

Gabby Burt

Gabby Burt, R.N., B.S.N., C.B.N.
Educator
Gabby is a registered nurse specialized in diabetes education. Her nursing experience includes medical surgical nursing and nurse coordination for weight loss surgery patients. Gabby’s passions include assisting patients with chronic conditions to optimize their health and improve their quality of life through group and individualized health education. She is working toward becoming a certified diabetes educator.

Leigh Clary

Educator
Leigh is a Registered Nurse educator with Health Management and Education. Leigh has worked as a nurse since 1997 in various settings including inpatient acute care, emergency department and outpatient education. Within the department she enjoys teaching self-management group health education classes as well as working individually with patients who have diabetes. She recognizes the challenges people face when managing chronic illnesses and believes education and support are key factors in allowing people meet their health goals. In 2017 Leigh became a Certified Diabetes Educator.

Kirsten Manley

Case Manager
Kirstin is a registered nurse who began working at UC Davis Health in 1996. She had experience in various settings in the hospital, including cardiology and pediatrics, before joining the Ambulatory Case Management team in 2012.

Sharon Myers

Sharon Myers, R.N., M.A., C.H.F.N.
Educator
Sharon is a Registered Nurse and is specialty certified as a Certified Heart Failure Nurse. She has her Master’s degree in Counseling & Psychology as well as Exercise Physiology. She has worked for UC Davis Health for over 15 years. Sharon is a lead facilitator of the heart failure self-managing class.

Annie Saechao

Annie has been a Registered Nurse since 2013.  She has worked in the acute care setting on a Cardiothoracic Progressive Care unit and as a discharge planner working with NICU and OB patients.  She joined the team in 2017 in the role of an RN Case Manager/Intake Referral Coordinator.

Cari Shulkin

Cari is a Registered Nurse, board certified in Cardiac Vascular Nursing and as a Tobacco Treatment Specialist. She has worked at UC Davis Health since 1990. She joined the education team in 2012 with her primary focus being tobacco cessation education as well as self-management of heart and lung disease.

Kate Van Wagenan

Kate is a Registered Nurse with Health Management and Education. She has worked as a nurse since 1981 in Arizona, Nevada and California in various settings including inpatient chronic and acute care, medical transport, and outpatient oncology, clinical trials, education, managed care and case management. She has been at UC Davis since 2011 in oncology clinical trials and the Managed Care Department and Health Management and Education Department since 2013.  She is aware of the many challenges in the health care system and is passionate about working individually with patients to meet their needs.  Kate wants patients to know the high standards and caring professionalism that UC Davis Health provides as well as the resources available to assist with the management of their health care.

Cynthia has worked as a Registered Nurse since 1978 in Sydney, Australia and California. She has experience working in a variety of settings including surgery, home health, hospice, discharge planning, case management, and education. In her Health Management and Education role, Cynthia enjoys problem solving, providing  resources, and helping patients navigate through UC Davis Health. She recognizes chronic illness is especially challenging and believes in supporting patients to help them achieve their optimum health status..

Linda Goode

Case Manager
Linda is a Licensed Clinical Social Worker who joined Health Management and Education’s Ambulatory Case Management team in 2013. Earlier in her UC Davis Health career Linda facilitated breast cancer awareness and education as a Senior Mammographer. She has worked at UC Davis Health for more than 25 years in various clinical capacities. In her role as a Clinical Case Manager Linda’s extensive experience allows her to address a variety of complex and diverse mental health conditions.

Daniel Gould

Case Manager
Daniel is a Licensed Clinical Social Worker and utilizes his more than 20 years of case management and clinical mental health experience in his role as a case manager. He has been a member of the Ambulatory Case Management team since its launch in 2012.

Katie Klein

Case Manager
Katie is a Case Manager with a Master degree in Social Work. Her career at UC Davis Health began in 2002 providing community case management and clinical case management for discharge planning. She joined Health Management and Education’s Ambulatory Case Management team in 2012. Her background and training bring valuable skills to her current role as a case manager, intake coordinator and resource expert. 

Stephanie Ladd

Case Manager

Stephanie is a Licensed Clinical Social Worker who joined the Health Management and Education team in 2017. She brings over 10 years of experience in clinical mental health. Prior to joining Health Management and Education Stephanie specialized in trauma and abuse, working with children and families to provide therapy and case management. Stephanie is also a veteran who retired from the U.S. Air Force Reserves in 2012.

Kristin is a Licensed Clinical Social Worker and has worked with UC Davis Health since 2008.  Joining the HME team in 2014 Kristin brings over 10 years experience in medical social work with a background in geriatrics, renal transplant, HIV/AIDS, and oncology.
