Medical Director
Dr. Hooper became the medical director of Health Management and Education (HME) in 2016. He has been with UC Davis since 1996, initially as a Family Physician in the Primary Care Network (PCN) and then transitioned to an administrative role in the PCN in 2012 when he also took on the role as Medical Director for UCD Managed Care. In his present position as the Medical Director for Care Services and Innovation, one of his many responsibilities is to oversee HME. Dr. Hooper believes an important part of his role is to support and help guide the efforts of the dedicated, talented group of professionals in the Health Maintenance and Education Department. Dr. Hooper considers the work in HME a very important part of what UCD is striving for -  to improve the treatment and health outcomes of the patients. As a Family Physician with experience in clinical practice for over 25 years,  he feels strongly that the primary care physician plays a key role in assuring that their patients receive high quality medical care across the spectrum of their life and the continuum of care. The primary care physician role has become increasingly complex for a variety of reasons especially caring for the sickest, most vulnerable patients managed by the primary care physicians. Dr. Hooper looks forward to the privilege of working with the HME team for the benefit of the patients cared for by all of our UCD physicians.

Judith Johnson

Judith Johnson, R.N., M.S.N., D.N.P., N.E.A.-B.C.
Director
Judith is the Director of Health Management and Education and has worked for UC Davis Health since 1988. She has been with the Primary Care Network since 2003, becoming the Director of HME in 2016. She brings extensive nursing experience including work with patients with chronic illnesses, culturally diverse populations, and those with psychosocial and mental health needs. As the new Director of Health Management and Education, Judith brings a dedication to patient centeredness and foresees the future growth of the HME team in continuing to promote quality care and care coordination while empowering patients with the tools they need for success and wellness.

Glee Van Loon

Glee Van Loon, R.D., C.D.E.
Education Supervisor
Glee is a registered dietitian and Certified Diabetes Educator (CDE) who has been with  UC Davis Health  and Health Management and Education (HME) department since 2007.  As part of HME’s leadership, Glee supervises the education team, contributes to the department’s overall growth and development and ensures the delivery of high quality education.  Glee has been a dietary specialist in diabetes management since 2003. She facilitates health classes and provides individual medical nutrition therapy.

Senior Community Health Program Representative
Derika has worked at UC Davis health since 2006 and has brought her diverse life coaching skills and Holistic Health background to the department in 2011. Currently, Derika provides support for daily Health Management and Education operations which includes office administration, providing resources to patients and staff, discussing education benefits with patients, and scheduling/ assisting self-management classes.

Kristina McCoy

Kristina has worked at UC Davis Health providing quality customer service and departmental support since 2002. She joined the department in 2011 as a Senior Community Health Program Representative, maintaining the daily operations of the education team and supporting the efforts of the Director. She coordinates and schedules self-management classes as well as assists the staff with project development and website support.

Educator
Judy joined UC Davis Health and education team in 2012 as a Community Service Case Manager and a Health Educator for chronic disease self-management classes. Her previous work in corporate health promotion brings value to the education team.

Moreen came to UC Davis Health and joined the education team in 2012. She is a Community Service Case Manager and a Health Educator for chronic disease self-management classes. Moreen is a Certified Health Education Specialist.

Health Psychologist

Debra Fishman

Debra Fishman, PsyD.
Educator
Dr. Fishman is a Psychologist who completed her clinical training as an Intern and Fellow with UC Davis, Department of Family and Community Medicine. She provides individual counseling and group education to patients with chronic health conditions. Dr. Fishman’s work is focused on helping patients learn about and integrate specific health self-management tools and strategies into their personal health management plan. She is valued for her broad background that includes training and work experience in Health Psychology, Exercise Physiology, Cardiac Rehabilitation, Ergonomics, and management of outpatient physical therapy serices. She is dedicated to providing patient-centered care.

Pharmacist

Linda Phan

Linda Phan, Pharm D., B.C.P.S.
Pharmacist
Linda has been a pharmacist for over 18 years, most of which have been spent at UC Davis taking care of hospitalized patients. Linda joined the HME team in 2014 to work more closely with patients on their needs beyond the hospital. In addition to her Doctorate in Pharmacy, Linda has additional training in both geriatric pharmacy practice and is currently board certified as a pharmacology specialist.

Huong Sandhu

Huong Sandhu, Pharm D., M.S.
Pharmacist
As a Care Coordination pharmacist, Huong’s experience includes expertise in long term care, managed care and clinical health education. She joined Health Management and Education in 2015 and is collaborating with an expanded care team to initiate an innovative program for guiding patients with high blood pressure to better control.

Marie Barone

Marie Barone, R.D., C.D.E.
Educator
Marie is a Registered Dietitian and Certified Diabetes Educator who has been with UC Davis Health since 2007. She joined Health Management and Education team in 2009. Serving as a nutrition expert for the team, she facilitates self-management classes for people with chronic conditions and provides medical nutrition therapy to patients individually. In addition, Marie is skilled in electronic education methods and takes a lead in program development for the education team.

Melinda Gong

Melinda Gong, R.D.
Educator
Melinda is a Registered Dietitian with a background in clinical dietetics and holds a certificate of training in Weight Management. She has been providing nutrition care for a variety of populations from pediatrics to geriatrics and serves as a nutrition expert to the education team. She joined the department in 2012 and is an educator for chronic disease self-management classes. She is working towards becoming a Certified Diabetes Educator.

Gabby Burt

Educator
Gabby is a registered nurse specialized in diabetes education. Her nursing experience includes medical surgical nursing and nurse coordination for weight loss surgery patients. Gabby’s passions include assisting patients with chronic conditions to optimize their health and improve their quality of life through group and individualized health education. She is working toward becoming a certified diabetes educator.

Leigh Clary

Leigh Clary, R.N., B.S.N., C.D.E.
Educator
Leigh is a Registered Nurse educator with Health Management and Education. Leigh has worked as a nurse since 1997 in various settings including inpatient acute care, emergency department and outpatient education. Within the department she enjoys teaching self-management group health education classes as well as working individually with patients who have diabetes. She recognizes the challenges people face when managing chronic illnesses and believes education and support are key factors in allowing people meet their health goals. In 2017 Leigh became a Certified Diabetes Educator.

Sharon Myers

Sharon Myers, R.N., M.A., C.H.F.N.
Educator
Sharon is a Registered Nurse and is specialty certified as a Certified Heart Failure Nurse. She has her Masters degree in Counseling & Psychology as well as Exercise Physiology. She has worked for UC Davis Health for over 15 years and joined the department in 2011. Sharon is a Heart Failure Care Manager, educating and coaching patients with heath failure.

Cari Shulkin

Cari is a Registered Nurse, board certified in Cardiac Vascular Nursing and as a Tobacco Treatment Specialist. She has worked at UC Davis Health since 1990. She joined the education team in 2012 with her primary focus being smoking cessation education as well as self-management of heart and lung disease.

Linda Goode

Linda Goode, L.C.S.W.
Care Manager
Linda is a Licensed Clinical Social Worker who in 2013 brought more than 22 years of UC Davis Health experience to the Care Coordination team. Her valuable history as a Clinical Case Manager allowed her to develop her expertise working with a variety of health conditions amongst a diverse patient population. Earlier in her UC Davis Health System career Linda facilitated breast cancer awareness and education as a Senior Mammographer.

Daniel Gould

Daniel is a Licensed Clinical Social Worker and utilizes his more than 22 years of case management and clinical mental health experience in his role as a care manager. He has been a member of the care coordination team since its launch in 2012.

Katie Klein

Health Service Navigator
Katie has worked for UC Davis Health since 2002. Previous roles include community case management and clinical case management/discharge planning. Her background and training bring valuable skills to her role as Health Service Navigator on the care coordination team, which she joined in 2012.

Kristin Quayle

Kristin is a Licensed Clinical Social Worker and has worked with UC Davis Health since 2008.  Joining the HME team in 2014 Kristin brings over 10 years experience in medical social work with a background in geriatrics, renal transplant, HIV/AIDS, and oncology.
