Care Management and Coordination

Care Coordination is a dynamic Health System program that identifies patient challenges and provides telephonic support to those with chronic conditions who need additional help as they manage their health at home. Through Care Coordination, organized care is provided to University of California Davis Health System patients by developing partnerships between the patient and their health care team.
How does Care Coordination work?
Care coordination improves patient experience and provides smooth transitions between the patient, health care team, and the patient’s medical neighborhood. Through doctor referrals and review of daily hospitalization, health service navigators reach out to patients who may have challenges stabilizing and maintaining their health. Based on identified needs, individual patients are assigned to a health care coach or care manager who provides telephonic support, motivation, and resources to meet patient centered health care goals and sustain lifestyle changes that improve quality of life.
