Carlos Quiroz.jpgTeresa Ogan, LCSW, is the Care Coordination Director for California Health Collaborative, a non-profit organization serving underserved and under-represented groups in California, and oversees the Multi-Purpose Senior Services Program, and Care Coordination Programs. Teresa has a Masters in Social Work from California State University – Sacramento and has over 20 years of experience in social services. Teresa has worked with children with special needs, children and families implementing wraparound services, and with seniors, caregivers and family members. Teresa has served on the planning committee for Honoring the Refugee & Immigrant Communities of Afghanistan, Iraq, and Syria that was hosted in November 2015, the California Department of Public Health’s Office of Health Equity Advisory Committee, and currently serves as a member on the Yolo Aging and Disability Resource Connection Advisory Committee and the Latino Health Community Advisory Board.
