CHeQ is leading the charge on the next wave of better management for high-impact diseases in California.
Currently, by the time providers are able to manually abstract data from charts and report it to public health using a paper-based process, often 24-36 months have passed since diagnosis. This slow and expensive process significantly and adversely impacts our understanding of disease trends. The data often exists in electronic format and could be reported more expediently. Ensuring a unified and complete record exists outside of local stovepipes will result in better quality of care for patients.

The Health Information Home (HIH) is a coordinated process enabling structured data capture from patient Electronic Health Records (EHRs) to transfer through Health Information Exchange (HIE) channels creating a complete, longitudinal record of history, diagnosis, plan, and treatment for high impact conditions such as cancer. This process will make a significant impact on care coordination, practice improvement, and disease registries.
