The UC Davis Asthma Network (UCAN) is an integrated clinical service established in 1999 by the Division of Pulmonary & Critical Care Medicine and the Division of Rheumatology, Allergy & Clinical Immunology as a resource to primary care providers to help manage poorly controlled asthma patients identified by primary care providers or health care resource utilization(s).  UCAN Clinics in Sacramento, Folsom and Davis offer asthma patients the convenience of one-stop service with evaluations of disease control and severity, treatment recommendations in an asthma action plan, and patient education.  Emergency Care Services Coordination for adult & adolescent patients by telephone pager 24/7/365.  This comprehensive multi-disciplinary chronic disease management program has markedly improved the quality of life of asthma patients while dramatically reducing emergency room visits and hospitalizations:
