Home Health program
Home health provides health-care services at home for people of all ages. We also offer patient and family education designed to enhance client health and independence. Active participation in the treatment plan is encouraged by both patients and caregivers to the fullest extent possible. Home-health services range in duration from a single visit to ongoing treatment over several weeks or months and are available to people in the greater Sacramento area.

Our professional home health nurses, social workers and therapists offer patient assessment, case management and a full spectrum of adult, geriatric, pediatric and maternal/child home care services.
