• This is an online course requiring an e-mail address, access to a computer and the Internet.  To register,  you must submit two application forms and the course fee of $100.  Please make checks or money orders payable to Old Marshall Adult Education.  It is important to include your telephone number and driver's license number on your check.   
• Your applications and payment  must be received prior to the acceptance deadline of August 1, 2009

For acceptance into Basic CPT/ICD-9 Coding, a certificate of completion or transcript of medical terminology as well as college-level transcript demonstrating successful completion of anatomy and physiology or proof of passing the UC Davis Health System's challenge exam(s) must accompany your registration form. If this documentation is not included, the form will be returned to you.
