Children born at UCDMC in Sacramento would obtain a birth certificate from the Sacramento County’s Clerk/Recorder. While UCDMC maintains medical records, we do not maintain birth certificates. All birth certificates are maintained by county government offices in the county where the child was born.

Patients requesting copies of their own records will take no longer than 15 days to process. All other requests will take no longer than 30 days to process.

For patients there may be a charge of .25 cents for each page plus the cost for shipping and handling.

Please contact Healthport, our in-house copy service, at 800-367-1500

You may email hs-roi@ucdavis.edu or call 916-734-5205 to check the status.  Please be sure to include name and date of birth so that we may better help with your request.
