Requesting Medical Records

 

Q: How do I request my medical records?

A: A written authorization form must be submitted to the Health Information Management. Click here to download and print out the form. The form can be filled out at our location, mailed, faxed, or scanned and sent via email. Please use the contact information below.

Q: Where can I get the authorization for release form?

Q: How long will it take to process my record request?

A: Patients requesting copies of their own records will take no longer than 15 days to process. All other requests will take no longer than 30 days to process.

Q: How much will it cost to receive my medical records?

A: For patients there may be a charge of .25 cents for each page plus the cost for shipping and handling.

Q: How can I check the status of my medical record request?

A: You may email roi@ucdmc.ucdavis.edu or call (916) 734-5205 to check the status.

Q: My doctor needs my records urgently, how can she get them?

A: We need to receive a request on letterhead from your physician which indicates which records are needed. The request needs to be faxed to (916) 734-8471. These requests are processed immediately, 24 hours a day.

A: No, due to the confidentiality of the records we are not able to fax records to you.

Q: How can I send other records to be included with my UC Davis Health System records?

A: You can mail the records to us, or fax them to (916) 734-0605.

Q: Can I review the official copy of my medical chart in person?

A: Yes, we can schedule an in person review for you. Click here to download our authorization form. Click here for instructions on submitting the form. Once the form is submitted we will contact you to schedule an appointment.

Q: Can the request for my family's medical records be on one request form?

A: No. A separate form must be completed and signed for each family member. For those family members who are 18 years of age or older, legal documentation must be provided when signing on behalf of the patient.

Q: Can I access my medical records over the Internet?

A: Yes. You can view your medical records and even contact your Physician using our MyChart service. Click here to sign up for MyChart.

Q: Can my Physician access my medical records via the Internet?

A: Yes. Your Physician can quickly and easily gain access to your medical records using our PhysicianConnect service. Click here for more information regarding PhysicianConnect.

Q: I found an error in my medical records. How can I have it corrected?
