Applications for the class entering in 2009

To obtain an application from April to October

  1. Download application and supporting documents to your personal computer, and
  2. Complete the form, save to your computer, print and mail it in with all supporting documents in one envelope.

Tips for a successful application

  • Read the application instructions and information brochure before completing the application;
  • Begin the search for a preceptor early, as this is sometimes the most difficult phase of the application;
  • Order two sets of transcripts: one set for you, and one set to submit with your application;
  • Contact three professional references who can attest to your patient-care and/or professional abilities;
  • Applicants are responsible for complying with admission requirements, prerequisites, following procedures and meeting all deadlines.
