Application Procedure
The UC Davis Medical Center Dietetic Internship Program uses the Dietetic Internship Program Application prepared by the American Dietetic Association (ADA) and Dietetic Educators of Practitioners (DEP) practice group. The application can be downloaded from the link above or the DEP Web site at www.depdpg.org.
In addition, the UC Davis Medical Center Dietetic Internship Program requires supporting materials to be sent in with the completed Dietetic Internship Program Application. Please read the following instructions and refer to the UC Davis Medical Center Dietetic Internship Application Checklist for specific application packet requirements. All materials in the application packet should be typed.
Instructions
- Application Deadline: To be considered for admission, all application material must be sent in one packet and postmarked no later than February 16, 2010, as determined by the ADA. If you wish acknowledgement that the application has been received, include a self-addressed stamped postcard which will be mailed back to you. Applications sent via Federal Express or other delivery services are accepted during non-holidays and Monday through Friday only.
- Application Fee: Enclose a check for $50.00 made out to “UC Regents.” This fee covers the processing of your application and is non-refundable.
- Transcripts: Original transcripts are required for all DPD courses, regardless of the number of schools attended. Photocopies of transcripts are not acceptable. Information and grades for fall courses should be included on one of the following:
- transcripts provided,
- fall grade sheet or
- a list of courses and their grades signed by your advisor.
- Three Letters of Recommendation: Three recommendations are required and only three will be considered.
- Include at least one ADA/DEP Recommendation Form or a letter from an academic contact (this may be your Dietetic Program Director, professor or academic advisor).
- The remaining ADA/DEP Recommendation Forms or letters may come from any of the following: an employer or someone who can evaluate you as your supervisor (this can be from paid or volunteer work experience) or from another professor in your major subjects.
- The ADA/DEP Recommendation Form should be used for all recommendations. The ADA/DEP Recommendation Form can also be downloaded from the DEP Web site at www.depdpg.org. In addition to the rating check boxes, please ask the writer to include a written summary of your scholarship achievements, ability to work collaboratively with others, leadership activities and/or ability to problem solve. This written summary can be included on the ADA/DEP Recommendation Form or in a separate letter.
- Include at least one ADA/DEP Recommendation Form or a letter from an academic contact (this may be your Dietetic Program Director, professor or academic advisor).
- A “Verification statement” or “Declaration of Intent to Complete Degree and/or CADE-Approved Minimum Academic Requirements", signed by you and your academic program director must be included in your application packet.
- Didactic Program in Dietetics (DPD) Course List: Include an official DPD course list developed by your college/university or DPD program (a photocopy of this list is acceptable). DO NOT utilize the DPD Course List section of the Dietetic Internship Program Application as a substitute.
- Substitute DPD Course Clarification List: The Substitute DPD Course Clarification List is REQUIRED when DPD courses are taken at more than one college or university. It MUST be included in your application to clarify courses and course numbers that do not match the official DPD Course List from your DPD program. Click this link to access a blank form: Substitute DPD Course Clarification List.
- Statement of Intent: In lieu of the traditional "Statement of Intent", the UC Davis Medical Center Dietetic Internship Program requests each applicant to answer six questions developed by the UCDMC DI program administrators. Click here to access Statement of Intent Questions. Retype the questions with your answers. Please answer the questions completely and limit each answer to ½ page, single-spaced.
- Resume: Provide a copy of your resume highlighting dietetics related and non-dietetics related experiences.
- Phone number and address: Include a phone number and address, on a card/paper separate from the application, where you can be reached on appointment day.
- Sign, date, and include the UC Davis Medical Center Dietetic Internship Application Checklist in your application packet.
- Self-Addressed Stamped Postcard (SASP) - optional: This is the preferred and most accurate method to ensure your application is received. The US Postal Service "return-receipt" is not necessary and has the potential to be an inaccurate assurance.
- After placing all requested materials, (those listed above in addition to the Dietetic Internship Program Application and the UC Davis Medical Center Dietetic Internship Application Checklist) in a manila file folder with your name and school written on the tab, put the file folder in preferred mail package and send to:
