Opening the form in

Microsoft Word so you can fill in the fields on your computer is a two-to-three step process.
- Step 1: Click on the Microsoft Word link.
- Step 2: Usually a "Save As ..." box will pop up, or you may have the option of saving or opening the document. Choose open if you have that option. Otherwise, choose the directory where you would like to save the file and click "save." Make note of the filename and location where you are saving the file so that you can find it later.
- Step 3: If the file is not already open, open the file you have just downloaded by double-clicking the file or opening Microsoft Word and opening the file.
The forms are compatible with Microsoft Word versions 6.0 and above.
