Application for continuing education course approval
Please follow these steps to request continuing education units for your course.
- Download the application:

- We recommend completing the application in Microsoft Word. If you would prefer to complete the form by hand, choose Adobe Reader. This includes all neccessary forms including except the course evaluation.
- Complete the educational design documentation:

- All speakers must complete a biographical data form.
- All persons directly involved in the planning and presentation must complete a planner/presenter/author Conflict of Interest Disclosure.
- Sending the file
- We recommend sending the file as an e-mail attachment to: diana.pearson@ucdmc.ucdavis.edu. If you need assistance with sending an attachment, please call the help desk (4-HELP). If you do not want to send the file as an e-mail attachment, please contact Diana Pearson or Patricia Campbell.
