UC Davis Chronic Disease Management (CDM) has created this Web site for your use in treating patients with chronic disease. The site has been developed in response to physician requests and will keep growing to increase the ease of providing guideline based chronic disease care which supports patient self-management.
Our mission has been focused on supporting the care of diabetes, asthma and heart failure. In addition to learning more about chronic disease management activities throughout the health system, you can learn how to refer your patients for self-management education classes as well as obtain self-management educational materials.
Since a big challenge for the health care system is making sure that each person gets the kind of help that they need to effectively deal with their condition, CDM has provided a Web site for patients. The site offers information about classes, educational programs and materials to help patients understand their condition so they can be in charge and in control.
