Who is CDM?

Bridget Levich, RN, MS, CDE
Director of Chronic Disease Management
Bridget is the director of Chronic Disease Management. She is a clinical nurse specialist and certified diabetes educator. In addition to managing the operations of the department, she is also part of the educational team for In Charge and In Control and is the facilitator for the Recharge Workshop. Bridget also works individually with patients in Insulin Therapy Evaluation appointments.
Marie Barone, RD
Registered Dietitian
Marie is a registered dietitian for the Chronic Disease Management team. She is an instructor for the In Charge and In Control and Dining with Diabetes classes. Marie is also a member of the Planned Visit education team.
Candace Carlson
Community Health Program Representative
Candace is Chronic Disease Management's Community Health Program Representative. She helps coordinate and schedule the self-management classes, and acts as a resource for both patients and health system staff. Candace is also an instructor for the Let's Get Started diabetes self-management classes and occasional educator for the Planned Visits.
Gail Shamberg RN, BSN, CCM, CRRN
Clinical Resource Nurse
Gail is a registered nurse with specialty certifications in Case Management (CCM) and Rehabilitation Nursing (CRRN). She helps manage the education curriculum and teaches several of the diabetes self-management group classes, including In Charge and In Control and Let's Get Started. Gail also coordinates Planned Visits, a program that combines a focused doctor's visit with point-of-care education, at the UC Davis clinics.
Larry Taylor
Analyst
Larry's role in the team is to prepare and distribute reports related to chronic disease activities. Additionally, Larry is responsbile for preparing reports for the Ambulatory areas of the health system.
Glee Van Loon, RD, CDE
Registered Dietitian
Glee is a registered dietitian and certified diabetes educator for the Chronic Disease Management team. In her role, she is part of the team who manages the education curriculum and teaches several of the diabetes self-management group classes including In Charge and In Control and Dining with Diabetes. She is out and about in the UC Davis Medical Group clinics regularly as an integral member of the Planned Visit education team which offers empowered learning to patients in their primary care physicians' offices. She also spends time promoting the work of Chronic Disease Management through marketing efforts.
