Upon written, signed request (E-mail not accepted, U.S. postal or fax accepted), an official transcript of the UC Davis Health System Coding Education & Training Program permanent student record will be mailed to any institution or person designated by the student. Unofficial transcripts cannot be provided.

The first two transcripts are issued free of charge. A fee of $2.00 is charged for each additional transcript requested. There is a $10.00 fee per copy for 24-hour transcript service and other fees may apply.
