Application Check Off List (only completed applications will be considered):

	Completed application
	Anatomy & physiology, official transcript or proof of good standing, or pass the challenge exam.
	Medical terminology, official transcript or proof of good standing, or pass the challenge exam.
	Copy of Basic Coding certificate or copy of program completion, or pass the challenge exam.
