Submit a Sample

Sample submission instructions

Submit a request:

1. Download the appropriate service request form:
• Nucleic Acids Sample Submission Form
• Sequencing Sample Submission Form

2. As described above under “Payment Terms”, orders for services are initiated and submitted via the web-based PPMS. This is utilized for orders from all clients and GSR personnel will be happy to assist in setting up PPMS accounts and order submission.
3. For requests that include bioinformatics needs, clients must complete the following form and submit along with the service request form:

• Bioinformatics Request Form
4. Complete all required fields electronically.
5. Email the completed form(s) as an attachment to gsr@ucdavis.edu.
6. The Genomics Shared Resource will follow up with you by email regarding your request.
