Pharmacy Anticoagulation Services
UC Davis Medical Center Pharmacy Anticoagluation Services cares for patients who are at risk for blood clots or stroke and who are taking drugs such as warfarin, direct oral anticoagulants (apixaban, rivaroxaban, edoxaban, dabigatran), and low molecular weight heparin (enoxaparin and dalteparin). The anticoagulation staff consists of an interdisciplinary team of pharmacists, physicians, and medical office service coordinators specially trained in anticoagluation therapy. We collaborate with other health care professionals to ensure patients recieve safe and effective care while being on anticoaglulation therapy.
Our staff assists with monitoring lab results in the inpatient hospital and outpatient clinic setting to assess if any changes need to be made in the dosing of anticoagulation therapy. Patients seen in clinic are consulted over the phone or seen face-to-face at our clinic, recommending dose changes and follow-up lab work. Click here to learn more about what we can do for you.
