Welcome to the University of California, Davis Anticoagulation Program

This program covers both the inpatient (hospital) setting and the outpatient setting.  The program covers approximately 50 patients per day in the hospital and 2200 patients in the outpatient clinic. 

Our program is staffed by pharmacists who are specifically trained to handle complex anticoagulation issues along with friendly staff ready to assist you with any problem that might arise.  The program includes a discharge program to enable patients a smooth transition from the inpatient to the outpatient setting, face to face visits and a comprehensive telephonic program that allows patients to be treated right from the laboratory in their neighborhood.

The clinic is located adjacent to the hospital at 4860 Y Street, Sacramento, CA 95817.  Our office hours are 8:00 a.m.–4:30 p.m. Monday through Friday and closed for lunch from noon–1 p.m.  Please feel free to call us at (916) 734-8158 or visit the many resources available on this website.
